Zusammenfassung

Die Paukendrainage ist der häufigste chirurgische Eingriff in Deutschland überhaupt mit dem Ziel der Gewährleistung von Mittelohrventilation und Sekretabfluß aus der Paukenhöhle sowie sofortiger Hörverbesserung und Vermeidung von Komplikationen und Spätfolgen wie Trommelfell-, Paukenhöhlenpathologien und Entwicklungsbeeinträchtigungen. Die volkswirtschaftliche Bedeutung wird dadurch unterstrichen, dass in Deutschland etwa jährlich 300 000 Parazentesen durchgeführt werden. Solange die benötigte Mittelohrventilation nicht vorhersagbar und ideale Materialien und Techniken für das Offenhalten nicht gefunden sind, bleibt die Art der Paukendrainage eine immer wieder neu zu treffende individuell an den Patienten anzupassende Abwägungsentscheidung. In diesem Zusammenhang werden die verschiedenen Techniken der Paukendrainage und Mittelohrventilation wie konventionelle Parazentese, Laser-Parazentese, Kurz- und Langzeitpaukenröhrcheneinlage sowie Indikationen, Komplikationen und deren Behandlung beschrieben.

Abstract

Tympanostomy is the most frequently performed surgery procedure in Germany. It enables middle ear ventilation and discharge of secretion from the middle ear with immediate hearing improvement and avoidance of complications and long-term pathologies of the tympanic membrane and the tympanum. The economical importance is emphasized by the fact that about 300 000 myringotomies are performed yearly. The best type of middle ear drainage and ventilation should be chosen on a case-by-case basis. In this context, the various techniques like conventional myringotomy, laser-myringotomy, short-term and long-term tympanostomy tubes and indications, complications and their treatments are described.
