Patientenfragebogen zum Formular 61 nach den Rehabilitationsrichtlinien des Gemeinsamen Bundesausschusses (PF61R) – Entwicklung und Praktikabilitätstestung

Zusammenfassung

Seit der Verabschiedung der „Richtlinie des Gemeinsamen Bundesausschusses über Leistungen zur medizinischen Rehabilitation (Rehabilitations-Richtlinie)“ im Jahr 2004 gilt in Deutschland ein neues Verfahren zur Einleitung der medizinischen Rehabilitation im Auftrag der Krankenkasse. Diese Richtlinie beruht auf der ICF-Klassifikation der WHO und regelt in 2 Schritten das Antragsverfahren. Als ersten Schritt weist der Arzt durch das Formular 60 die Krankenkasse auf einen möglichen Rehabilitationsbedarf des Patienten hin; die Krankenkasse beauftragt ggf. in einem zweiten Schritt den Arzt, eine „Verordnung von medizinischer Rehabilitation“ (Formular 61), auszustellen. Das Ausfüllen dieser 4-seitigen Verordnung bzw. das damit verbundene Patientengespräch und die Aktenrecherche sind oftmals mit einem hohen zeitlichen Aufwand verbunden. Um den Ärzten das Ausfüllen des Formulars 61 nach der Rehabilitations-Richtlinie zu erleichtern und die Qualität der Anträge zu verbessern, wurde von der Projektgruppe Rehabilitation des Regionalen Kooperativen Rheumazentrums Hannover e. V. in Zusammenarbeit mit der Koordinierungsstelle Angewandte Rehabilitationsforschung und der Klinik für Rehabilitationsmedizin ein auf ICF Core Sets basierter Patientenfragebogen entwickelt und auf seine Praktikabilität getestet. Diese Testung erfolgte u. a. durch einen Evaluationsbogen, der sich sowohl an Patienten als auch an die involvierten Ärzte richtete. Die Ergebnisse dieser Begleitevaluation belegen, dass der Patientenfragebogen das Ausfüllen des Formulars 61 erleichtern und dessen Qualität verbessern kann und der Einsatz in der Praxis von den Ärzten mit einer deutlichen Mehrheit von 98% befürwortet wird.

Abstract

Since the passage of the Guidelines of the Federal Joint Committee on Benefits for Medical Rehabilitation (“Rehabilitation Guidelines”) in 2004, a new procedure for the approval of medical rehabilitation as a health insurance benefit has been in effect in Germany. The guideline, which is based on the International Classification of Functioning, Disability and Health (ICF) of the World Health Organization, provides for a 2-step application procedure. Step 1: The physician submits Form 60 to the health insurance company to inform the insurer of the insured’s potential need for rehabilitation. Step 2: If deemed appropriate, the health insurance company authorizes the physician to issue an “order for medical rehabilitation” (Form 61). The work required for completion of this 4-page document, including the necessary consultation with the patient and review of the patient records, is often very time-consuming. To facilitate the completion of Form 61 according to the Rehabilitation Guideline and to improve the quality of applications, a patient questionnaire based on ICF Core Sets was developed and tested for practicability by the Rehabilitation Project Group of the Hannover Regional Cooperative Center for Rheumatic Conditions in collaboration with the Coordinating Centre for Applied Rehabilitation Research and the MHH Department of Rehabilitation of Medicine in Hannover, Germany. Testing was carried out using various instruments, including an evaluation form for the involved patients and physicians. The results of the accompanying evaluation show that the patient questionnaire facilitates the completion and improves the quality of Form 61. The overwhelming majority of physicians (98%) were in favor of using the questionnaire in practice.
