Gallbladder Hemorrhage Mimicking Acute Cholecystitis in a Patient under Antiplatelet Therapy

Abstract

Hemorrhage within the gallbladder is rare but potentially fatal. It can be easily overlooked and misdiagnosed because of its low incidence and presentation that is similar to other types of hepatobiliary disease. We present an unusual case of gallbladder hemorrhage in a 36-year-old male patient who was taking antiplatelet medication following cardiac surgery. The patient was misdiagnosed with acute primary cholecystitis and, in consideration of postoperative complications with cardiovascular comorbities, serial treatment with urgent percutaneous transhepatic gallbladder drainage and delayed laparoscopic cholecystectomy was prescribed. Unexpectedly, a gallbladder hemorrhage was intraoperatively confirmed as the gallbladder appeared filled with dark blood clot-like materials. The patient recovered well and was discharged on the 3rd postoperative day without complications. In patients who are on antiplatelet therapy, gallbladder hemorrhage should be considered in the differential diagnosis if they have symptoms typical of biliary colic. In addition, delayed surgery with prompt biliary diversion may represent a safe and effective treatment approach.

Zusammenfassung

Blutungen in der Gallenblase sind selten, aber können unter Umständen schwerwiegend sein. Aufgrund geringer Inzidenz und ihrer Ähnlichkeit im Auftreten mit anderen Leber- und Gallenkrankheiten sind sie leicht übersehbar und werden häufig falsch diagnostiziert. Wir stellen hier einen ungewöhnlichen Fall von Gallenblasen-Blutung anhand eines 36-jährigen, männlichen Patienten, der vor der Herzchirurgie gerinnungshemmende Medikamente einnahm, vor. Dem mit einer akuten, primären Cholezystitis falsch diagnostizierten Patienten wurde unter Berücksichtigung von postoperativen Komplikationen mit kardiovaskulärer Komorbidität eine dringende serielle Behandlung von perkutaner transhepatischer Gallenblasendrainage und eine verzögerte laparoskopische Cholezystektomie verordnet. Unerwartet wurde eine Gallenblasenblutung interoperativ bestätigt, als sich die Gallenblase mit einem dunklen, starken Blutgerinnsel zeigte. Der Patient erholte sich gut und wurde an dem 3. Tag nach der Operation ohne Komplikationen entlassen. Bei Patienten, die unter einer antikoagulatorischen Therapie stehen, sollte bei der Differenzialdiagnose eine Gallenblasen-Blutung berücksichtigt werden, wenn sich Symptome einer typischen Gallenkolik aufzeigen. Außerdem kann eine verzögerte Operation mit einer schnellen Gallenableitung eine sichere und effektive Behandlungsmöglichkeit darstellen.
