Clomifenstimulation in der Praxis

Kernaussagen

  • Clomifen ist nach wie vor das Medikament der ersten Wahl zur Ovulationsinduktion bei anovulatorischer Infertilität und auch PCOS. Eine zusätzliche Indikation bei ovulatorischen Zyklen besteht zur ovariellen Stimulation vor geplanter Inseminationstherapie.

  • Üblicherweise wird mit der Einnahme einer Tablette (50 mg) vom 3.–7. Zyklustag begonnen, allerdings ist auch ein späterer Therapiebeginn im Zyklus insbesondere bei Vorliegen einer Amenorrhö und fehlendem Nachweis größerer Follikel sowie einem flachen Endometrium ohne Einschränkung des therapeutischen Erfolges möglich.

  • Eine Dosissteigerung sollte bei initial fehlendem Ansprechen maximal auf 150 mg über 5 Tage erfolgen, und die Zahl der konsekutiv durchgeführten Therapiezyklen auf maximal 6 begrenzt sein.

  • Wegen der ca. 14 %igen Wahrscheinlichkeit einer polyfollikulären Reifung auch bei niedriger Clomifendosierung sollte jeder Clomifenzyklus durch ein Monitoring begleitet werden, um das Risiko einer Mehrlingsschwangerschaft zu minimieren.

  • Das sonografische Monitoring verbunden mit der zusätzlichen Gabe von hCG (5000 IE oder 250 µg) bei Nachweis von mindestens einem Follikel > 17 mm und die Beratung zum Konzeptionsoptimum ca. 24–36 Stunden nach hCG-Gabe ergibt eine maximale therapeutische Effektivität.

  • Bei etwa 25 % der Patientinnen ist mit dem Vorliegen einer Clomifenresistenz zu rechnen. Etablierte Second-Line-Therapieoptionen sind der Übergang auf eine Gonadotropinstimulation oder das Drilling der Ovarien im Falle eines PCOS.

  • Bei erhöhtem BMI kann eine Patientin mit PCOS und Clomifenresistenz das ovarielle Ansprechen durch Gewichtsreduktion und Lebensstilmodifikation signifikant verbessern.

  • Bei erhöhtem BMI, PCOS und zusätzlich nachgewiesener Insulinresistenz ergibt sich die therapeutische Option für eine Metformingabe im individuellen Heilversuch.

  • Der Einsatz von Aromatasehemmern scheint nach der aktuellen Studienlage äquieffektiv zu Clomifen und könnte zukünftig auch eine First-Line-Therapieoption alternativ zu Clomifen darstellen. Aktuell überwiegen allerdings die breiten Erfahrungen mit Clomifen, sodass die mögliche Indikation für Aromatasehemmer bei einer Clomifenresistenz zu sehen ist.
