In der internationalen Klassifikation psychischer Störungen (ICD-10) versteht man unter einer Dysthymia eine anhaltende depressive Verstimmung mit einem Krankheitsverlauf von mindestens 2 Jahren, die im Hinblick auf Schweregrad und Dauer einzelner Episoden nicht die Kriterien einer leichten, mittelgradigen oder schweren rezidivierenden depressiven Störung (ICD-10 F33) erfüllt. Oft durch einen frühen Beginn im Erwachsenenalter gekennzeichnet, kann die Erkrankung Jahre andauern und ggf. lebenslang persistieren. In Deutschland wurden Lebenszeitprävalenzen von bis zu 4 % berichtet. Bei der Dysthymia ist eine Wirksamkeit sowohl der antidepressiven Psychopharmakotherapie als auch der Psychotherapie belegt. Dabei konnte hinsichtlich der antidepressiven Wirkung kein Unterschied zwischen den einzelnen Substanzgruppen der Antidepressiva gefunden werden, sodass bei der Auswahl andere Kriterien, wie die Verträglichkeit, pharmakokinetische Aspekte, Medikamentenwechselwirkungen, Überdosierungssicherheit und Handhabbarkeit, entscheidend sind. Neueste Arbeiten liefern Hinweise, dass eine alleinige Psychotherapie bei Patienten mit Dysthymia weniger wirksam ist als eine alleinige Psychopharmakotherapie. Im Rahmen einer gemeinsamen Empfehlung für chronisch verlaufende Depressionsformen unter Einschluss von Dysthymia, Double Depression und chronischer Depression weisen die aktuellen Leitlinien darüber hinaus auf eine Überlegenheit einer kombinierten Psychopharmako- und Psychotherapie gegenüber einer Monotherapie hin.
