Zusammenfassung

Zeitlich verzögert zur Etablierung einer evidenzbasierten Medizin (EbM) wird auch in der Ernährungswissenschaft der Ruf laut, Wirkaussagen mittels des von der EbM entwickelnden Evaluationsinstrumentariums zu bewerten. Eine derartige Entwicklung wirft bei näherer Betrachtung eine Reihe von Schwierigkeiten auf. Probleme bereiten dabei u. a. die Definition von „Evidenz” sowie die Begründung von Evidenzhierarchien und die Frage, welche Studientypen bei der Absicherung von Wirkaussagen im ernährungswissenschaftlichen Bereich zur Anwendung kommen sollen. Problematisch ist auch die Tatsache, dass das originäre EbM-Konzept mitunter nicht hinreichend bekannt bzw. fehlinterpretiert wird. So ist darauf hinzuweisen, dass eine Wirkaussage mitnichten nur dann als „evidenzbasiert” einzustufen ist, wenn sie sich auf eine randomisierte Interventionsstudie (RCT) bzw. einer darauf basierenden Metaanalyse stützen kann. Entsprechend wäre auch die Auffassung verfehlt, andere Studientypen mit niedrigeren Evidenzstufen besäßen nur eine untergeordnete oder gar keine Relevanz. Tatsächlich geht es bei der Erarbeitung evidenzbasierter Bewertungen nicht darum, diese auf der Basis der theoretisch bestmöglichen, sondern auf die der gegenwärtig bestverfügbaren Evidenz vorzunehmen. Im Sinne einer evidenzbasierten Ernährungswissenschaft plädieren die Autoren in Anlehnung an die PASSCLAIM-Kriterien (Process for the Assessment of Scientific Support for Claims on Foods) dafür, unter „bestverfügbare Evidenz” die Gesamtheit aller direkten und indirekten Belege (klinisch-epidemiologische Daten, biochemische und physiologische Befunde) die für oder gegen eine Wirkaussage sprechen, zu verstehen. Eine solche umfassende Gesamtbewertung, wie sie auf Basis der Hill-Kriterien vorgenommen werden kann, wirkt der eindimensionalen Evidenzklassifizierung von Evidenzhierarchien entgegen und vermeidet die einseitige Priorisierung klinisch-epidemiologischer Daten bei der Entscheidungsfindung. Evidenzgraduierungssysteme, wie sie etwa vom WCRF (World Cancer Research Fund) oder der WHO / FAO (World Health Organization / Food and Agriculture Organization) entwickelt wurden und sich an die Hill-Kriterien anlehnen, scheinen daher für ernährungswissenschaftliche und ernährungsmedizinische Fragen besser geeignet zu sein als gängige Evidenzhierarchien der EbM.

Abstract

After evidence-based medicine (EBM) has established itself, the demand for statements on efficacy using the evaluation tools developed by EBM has, with some delay, now also been raised in nutritional science. On closer consideration, such a development causes several difficulties. Among others, problems emerge with regard to the definition of „evidence” as well as the rationale for evidence hierarchies, and the question arises what types of study should be made in order to verify statements on efficacy in the field of nutritional sciences. Another problem is that sometimes the true EBM concept is not sufficiently known, or falsely interpreted. Thus, it should be pointed out that a statement on efficacy is not by any means to be considered „evidence-based” only if it is supported by a randomized intervention trial (RCT) or a meta-analysis based on RCTs. Accordingly, it is wrong to maintain that other types of study with lower strength of evidence would be of inferior relevance or none at all. An evidence-based evaluation should therefore not be based on the best evidence theoretically possible, but on the best evidence currently available. For the purpose of evidence-based nutritional sciences, the authors following the PASSCLAIM (Process for the Assessment of Scientific Support for Claims on Foods) criteria suggest that „best available evidence” should be understood to be the entirety of all direct and indirect supporting data (clinical-epidemiological data, biochemical and physiological results) speaking in favor of or against a statement on efficacy. Such a comprehensive overall evaluation which can be performed based on the Hill criteria, resists the one-dimensional classification of evidence in the form of evidence hierarchies and avoids the one-sided prioritization of clinical-epidemiological data in decision-making. Quality grading systems for evidence as they have been developed, for instance, by WCRF (World Cancer Research Fund) or WHO / FAO (World Health Organization / Food and Agriculture Organization) following the Hill criteria, thus appear to be more suitable for the clarification of issues of nutritional sciences and nutritional medicine than the widely used evidence hierarchies of EBM.
