Schizoaffektive Erkrankungen sind zwar ein nosologisches Ärgernis, aber eine klinische Realität. Sehr viele Menschen sind davon betroffen. Man geht davon aus, dass 20–30 % aller sog. „endogenen Psychosen” als schizoaffektiv zu bezeichnen sind. Es sind lebenslange Erkrankungen, die von einer hohen Suizidalität begleitet werden. Obwohl ihre Therapie komplizierter ist als bei den „reinen Formen” (wie etwa Schizophrenien oder affektive Erkrankungen), haben sie eine deutlich günstigere Prognose als die Schizophrenien, aber eine etwas schlechtere als die affektiven Erkrankungen. Sie verlaufen in der Regel polyphasisch und sie benötigen eine praktisch lebenslange Betreuung.
