Zusammenfassung
Die perkutane Vertebroplastie und Osteoplastie sind minimalinvasive
palliative Therapieverfahren, bei welchen unter radiologischer Kontrolle
Knochenzement in eine schmerzhafte osteolytische Knochenmetastase eingebracht
wird. Neben der palliativen Schmerztherapie kommt der Stabilisierung
frakturierter Knochen und Wirbelkörper eine entscheidende Bedeutung zu.
Zur lokalen Metastasenbehandlung wird die Methode zunehmend erfolgreich mit der
Radiofrequenzablation (RFA) kombiniert eingesetzt.
Summary
Osteolytic metastases are a challenging problem in cancer patients.
Moreover, these patients are increasing because primary tumours are well
controlled by recent advances in primary cancer treatments. Nevertheless, pain
relief and bone stabilization are the priorities for most cancer patients with
painful bone metastases and pathological vertebral fractures. Until now,
treatments for bone metastases have been mainly conservative, such as radiation
therapy, chemotherapy, or analgetics. Additionally vertebroplasty and
osteoplasty has been widely used in these patients. Recently, a new technique
that combines RF ablation therapy followed by percutaneous cement injection has
shown to be effective when bone metastases become bulky, extend outside bones
or in metastastic lesions with epidural extension and cortical disruption.
