Das Mesangium besteht aus mesangialer Matrix und multifunktioneller Mesangiumzelle. Es stellt die Stützstruktur für die Architektur der Kapillarschlingen dar und kann durch eine erhöhte Produktion der Matrixproteine oder durch Mesangiumzellproliferation expandieren. IgA–Nephritis ist die häufigste Erkrankung des Mesangiums, die sich durch eine deutliche Zellproliferation aufgrund von IgA– und Komplement–Ablagerungen manifestiert und zu einer chronischen Entzündung führt. Bei der membranoproliferativen Glomerulonephritis geht eine mesangiale Zellzunahme mit einer Ablagerung von Immunkomplexen im Mesangium, in der glomerulären Basalmembranen unterhalb des Endothels und unterhalb der Podozyten einher; zusätzlich ist eine starke Monozyten–/Makrophagen–Infiltration intrakapillar und mesangial zu sehen. Die diabetische Glomerulosklerose ist histologisch durch eine Verbreitung der glomerulären Basalmembran und eine diffuse und/oder mesangiale Matrixvermehrung gekennzeichnet. Amyloidose und Leichtketten–Nephropathie können ebenfalls zu einer nodulären Verbreitung des Mesangiums aufgrund von Fibrillen– und Leichtkettenablagerungen führen und sollten in die Differenzialdiagnose der diabetischen Glomerulosklerose mit einbezogen werden.

The mesangium consists of mesangial matrix and multipotent mesangial cells. Function and plasticity of mesangial cells are crucial for maintaining normal glomerular structure. Most glomerular disorders target the mesangium. IgA nephritis is the most frequent mesangial disorder where, in a response to IgA and complement deposition, prominent cell proliferation and mesangial matrix increase can be seen. In membranoproliferative glomerulonephritis, deposition of immune complexes in mesangium, basement membranes, subendothelially and subepithelially is typically associated with pronounced glomerular monocyte/macrophage infiltration. Diabetic glomerulosclerosis appears as basement membrane and mesangial expansion. Amyloidosis and light chain nephropathy can also cause a nodular mesangial expansion that may be included in the differential diagnosis of diabetic glomerulosclerosis.
