Einleitung

Das diabetische Fußsyndrom (DFS) ist ein besonderes Problem unter den Komplikationen des Diabetes. Es ist nicht ausschließlich eine klassische Spätkomplikation, sondern kommt bereits bei neu diagnostizierten Diabetikern gehäuft vor. Erhöhte plantare Drücke in Kombination mit einer diabetischen Polyneuropathie und ggf. einer Angiopathie führen zusammen mit einer vermutlich systemischen Komponente (Störung der Wundheilung auf zellulärer Ebene) bei Patienten mit einem Diabetes mellitus zur chronischen Fußläsion.

Die Prävalenz schwankt zwischen 4 und 15 %. Neuere Daten lassen auf über 40 000 Amputationen oberhalb des Sprunggelenkes (sog. Major-Amputation) bei Diabetikern in Deutschland schließen, womit die Inzidenz von diabetesbedingten Amputationen in Deutschland höher liegt als im europäischen Vergleich. Besorgniserregend ist hierbei, dass ein relativ großer Anteil als Oberschenkelamputation erfolgt. Dies bedeutet für die meist älteren Patienten mit häufigen Komor­biditäten neben der psychischen Belastung eine geringere Mobilität und eine deutliche Steigerung der Mortalitätsrate [1] [2] [3] .
