Zusammenfassung
Ein bilaterales Aneurysma der A. carotis interna wurde bei einem 9jährigen Jungen
angiographisch nachgewiesen. Das Aneurysma der rechten A. carotis interna zeigte eine
extreme Ausdehnung von dem zervikalen Teil der Arterie durch den Canalis caroticus
bis in den Sinus cavernosus. Das linksseitige Carotis-interna-Aneurysma hatte eine
symmetrische Lage am Hals, war kleiner und nur auf den zervikalen Arterienabschnitt
beschränkt. In dieser Körperregion bestanden noch zwei weitere Anomalien: Blockwirbelbildung
C6 und C7 mit Assimilation der Wirbelbögen und Dornfortsätze, sowie eine hypoplastische
A. vertebralis sin. mit einem abnormen Abgang aus der Aorta. Das kongenitale doppelseitige
Aneurysma blieb bis zum Auftreten einer Oculomotoriusparese klinisch stumm.
Summary
Bilateral aneurysms of the internal carotid artery of a boy of 9 years of age are
reported. There was a giant aneurysm on the right internal carotid artery which is
spreading from the cervical portion of the vessel into the carotid canal and the cavernosus
sinus. A second great aneurysm of similar site was detected angiographically on the
left internal carotid artery but was limited only to the extracranial portion. There
were two other anomalies on the neck, a partial vertebral fusion with assimilation
of the vertebral arches and spinous processes of C-6 and C-7, secondly a hypoplastic
left vertebral artery with anomalous origin from aortic arch. There was no clinical
evidence of the congenital bilateral aneurysm until a right oculomotor palsy developed.
Résumé
Compte rendu d'un anévrysme bilatéral de la carotide interne chez un garçon de neuf
ans. L'un, géant, inséré sur la partie cervicale de la carotide interne droite, envahissait
le canal carotidien et le sinus caverneux. L'autre, symétrique du côté gauche, était
plus petit et se limitait à la partie cervicale de l'artère. Deux autres anomalies
étaient relevées dans la région: Une fusion partielle des corps vertébraux de C6 et
de C7, intéressant aussi les arcs et les apophyses épineuses, – et une hypoplasie
de l'artère vertébrale gauche qui prenait une origine anormale sur l'arc aortique.
Il n'y eut aucune manifestation clinique de ces anévrysmes congénitaux jusqu'à l'apparition
d'une parésie de la 3ème paire droite.
