Excision of Arteriovenous Malformation of the Vein of Galen Complicated by Congestive Heart Failure

Summary

Arteriovenous malformations of the great vein of Galen and particularly those causing congestive heart failure are extremely difficult to manage. Out of 27 such cases reported in the literature 25 died, the 2 that survived were operated.

The successful excision of the malformation in a 2 1/2-year-old child is outlined, and some hemodynamic mechanisms which may be responsible for the development of cardiac failure in association with cerebral arteriovenous malformations are briefly discussed.

Arteriovenöse Mißbildungen der Vena Galeni magna und besonders solche, die Herzfehler verursachen, sind äußerst schwierig zu behandeln. Von 27 solcher in der Literatur berichteten Fälle starben 25, die beiden Überlebenden wurden operiert.

Die erfolgreiche Exzision der Mißbildung bei einem 2 1/2 jährigen Kind wird beschrieben, und einige hämodynamische Mechanismen, die für die Entwicklung von kardialen Beschwerden in Verbindung mit zerebralen arteriovenösen Mißbildungen verantwortlich sein könnten, werden kurz skizziert.

Les malformations artério-veineuses de la grande veine de Galien, en particulier celles qui déterminent des lésions au coeur, sont très difficiles à traiter. Sur les 27 cas publiés dans la littérature, 25 ont abouti à la mort. On décrit l'excision avec succès d'une telle malformation chez un enfant de 2 ans 1/2 et on esquisse brièvement quelques mécanismes hémodynamiques auxquels pourraient être imputées les lésions cardiaques liées à des malformations artério-veineuses cérébrales.
