Die (Eisenmangel-)Anämie zählt bei Frauen im gebärfähigen Alter zu den häufigsten Diagnosen. Das Patient Blood Management (PBM) ist ein individuelles Behandlungskonzept, das auf Reduktion von Anämie, Vermeidung unnötiger Blutverluste und einen evidenzbasierten Einsatz von Blutprodukten abzielt. Gerade bei Frauen vor einem chirurgischen Eingriff und in der Schwangerschaft kann es dazu beitragen, Patientinnen optimal auf eine Operation oder die bevorstehende Geburt vorzubereiten.
Abstract
In women of reproductive age, particularly during pregnancy and childbirth, patient blood management (PBM) plays an important role in reducing the risk of anemia and hemorrhage. These conditions are leading causes of maternal morbidity and mortality worldwide. By promoting early detection and treatment of iron deficiency anaemia, the implementation of a holistic PBM programme enables better outcomes for both, the mother and child. Implementing PBM protocols in perioperative care of women and perinatal care of parturients is essential to enhancing women’s, maternal and neonatal health on a global scale.

Eine Eisenmangelanämie ist die häufigste Ursache einer Anämie bei Frauen.

Im Sinne des Konzepts des Patient Blood Managements sollte der Hb-Wert präoperativ bei Frauen bei 13 g/dl liegen, um ihnen ähnlich gute Voraussetzungen wie Männern zu ermöglichen.

Schwangere haben während der gesamten Schwangerschaft einen zusätzlichen Eisenbedarf von 1 g.

Bei einer Eisenmangelanämie sollte bei schwangeren Patientinnen ab dem 2. Trimenon eine i. v. Eisensupplementierung durchgeführt werden.

Bei Patientinnen zur Sectio caesarea und einem erhöhten Blutungsrisiko wird der Einsatz der maschinellen Autotransfusion empfohlen.
