Einstellung der Zahn-Lippen-Relation im parodontal geschwächten Gebiss mittels Intrusion der Front

Zusammenfassung

Die Parodontitis zählt als chronisch entzündliche Erkrankung des Zahnhalteapparates zu einer der am häufigsten bakteriell, inflammatorisch bedingten Erkrankungen im Erwachsenenalter. Als eine zu mehr als 50% auftretende Komorbidität ist die pathologische Zahnwanderung, die u. a. mit funktionellen Einbußen (insuffizienter Mundschluss, Sprache, Ästhetik) vergesellschaftet ist. Nach einer parodontologisch/kieferorthopädischen Therapie wird eine Wiederherstellung der Funktion und die Verbesserung der knöchernen Situation der bewegten Zähne erreicht.

In der Diagnostik bieten neue Methoden wie die Stereophotogrammetrie hervorragende Möglichkeiten die Bildgebung non-invasiv zu erweitern und den Behandlungsverlauf und die damit verbundene Verbesserung der Weichteilsituation zu beurteilen.

Für eine erfolgreiche Therapie ist die Intrusion und Retrusion der betroffenen Frontzähne mit individuell angepasster Biomechanik notwendig. Dabei sind zu Beginn der kieferorthopädischen Therapie entzündungsfreie parodontale Verhältnisse obligat. Mit Hilfe von segmentierten Apparaturen und skelettaler Verankerung ist eine kontrollierte, nebenwirkungsarme Intrusion, mit dem Ziel ein funktionelles und dentofaziales Gleichgewicht zu erreichen, schon in der Nivellierungsphase möglich.

Für eine erfolgreiche Therapie und eine gute Prognose mit Verbesserung bzw. Erhalt der Mundgesundheit ist eine interdisziplinär parodontlogisch/kieferorthopädische Versorgung notwendig.

Abstract

As a chronic inflammatory disease of the periodontium, periodontitis is one of the most common bacterial inflammatory diseases in adults. A comorbidity that occurs in more than 50% of the patients is pathological tooth migration, which is associated with functional impairments (insufficient mouth closure, speech, aesthetics). After periodontal/orthodontic therapy, the restoration of function and the improvement of the bony situation of the orthodontically moved teeth is achieved.

In diagnostics, new methods such as stereophotogrammetry offer excellent opportunities to expand imaging non-invasively and to assess the course of treatment and the associated improvement of the soft tissue situation.

Successful therapy requires the intrusion and retrusion of the affected anterior teeth with individually adapted biomechanics. At the beginning of orthodontic therapy, inflammation-free periodontal conditions are obligatory. With the help of segmented appliances and skeletal anchorage, a controlled intrusion with few side effects, with the aim of achieving a functional and dentofacial balance, is already possible in the levelling phase.

Interdisciplinary periodontal/orthodontic care is necessary for successful therapy and a good prognosis with improvement or preservation of oral health.
