This prospective cohort study shows that 15 of 25 patients (60 %) with cT2 esophageal adenocarcinoma (EAC) can be downstaged following endoscopic reassessment to cT1 EAC, allowing endoscopic resection rather than surgery. Reassessment requires endoscopic expertise and patients should be referred to a tertiary referral center when there is doubt.
