The organizers could welcome 3500 registered participants joining from more than 100 countries. For the first time the conference was held fully virtual, requiring considerable additional efforts to prepare the meeting since planning started in January 2020, more than a year ago. The consensus panel has been enlarged to 59 panellists with 21 new members since 2017, continuing the process of generation change and rotation. More emphasis has also been given to low and middle income countries with active contribution of André Ilbawi of the WHO Cancer Control Program during the conference and the consensus. It was Philip Poortmans from the University of Antwerp Belgium who received the St.Gallen Breast Cancer Award 2021 for his commitment to patients and clinical research as well as for his interdisciplinary contributions to the St.Gallen Breast Cancer Conferences and Consensus over many years.
