Atypische Infektionen unter Immunsuppression

ZUSAMMENFASSUNG

Rasches Handeln ist gefragt, wenn der Verdacht auf eine Infektion bei einem immunsupprimierten Patienten besteht. Wenn es sich nicht gerade um einen tropischen Keim von der Kreuzfahrt bzw. den multiresistenten Keim von einem der letzten Klinikaufenthalte handelt – der Erreger ist meist nicht das Problem! Es ist seine Präsentation, die Ausprägung seiner klinischen Manifestation und seine Reaktion auf die Therapie, die für uns zur Herausforderung wird. Ist stets die Veränderung der Immunreaktion durch immer selektiver wirksame Biologika das Thema? Oder müssen wir uns erst mit den Reaktionen des innaten und adaptiven Immunsystems vertraut machen, um zu verstehen, was wir sehen?

SUMMARY

We need to take rapid action if we suspect an immunocompromised patient of having an infection. Unless it is a tropical germ from the vacation cruise or a multi-resistant strain from one of the patient’s last hospital stays – the pathogen usually isn’t the problem! It is the way that the pathogen presents itself, the extent of its clinical manifestation and its reaction to treatment, which pose a challenge. Is the issue always the regulation of the immune reaction by biologics that are increasingly selectively effective? Or do we first have to understand the reactions of the innate and adaptive parts of the immune system in order to comprehend what we are seeing?
