Korrektur einer mandibulären Asymmetrie mit frontal offenem Biss mittels Frontzahnextraktion und Lingualtechnik

Zusammenfassung

Einleitung Bestimmte Dysgnathien sowie von der Norm abweichende Zahnformen stellen für Patienten nicht nur funktionelle Risiken, sondern auch ästhetische Probleme dar. Die vorgestellte Kasuistik beschreibt die Korrektur beider Problemfelder.

Kasuistik Präsentiert wird der Fall einer 61-jährigen Patientin mit Laterognathie, moderater Angle-Klasse III, frontalem Kreuz- und Kopfbiss, frontal offenem Biss und ausgeprägten Engständen sowie deren Korrektur mittels vollständig individueller lingualer Apparatur und Extraktion eines unteren Frontzahnes sowie nachfolgender konservierender Umgestaltung einiger Frontzähne durch den Hauszahnarzt. Um die Ästhetik im Bereich der Extraktionslücke zu optimieren, erfolgte die temporäre Applikation eines Pontics durch den Kieferorthopäden.

Schlussfolgerungen Bei geeigneten Behandlungsfällen kann eine Frontzahnextraktion im Unterkiefer die Therapie einer Asymmetrie sowie einer moderate Angle-Klasse III mit frontalem Kopf-/Kreuzbiss unterstützen. Auch komplexe Fälle lassen sich mit einer vollständig individuellen lingualen Apparatur erfolgreich und vorhersagbar behandeln.

Abstract

Introduction Malocclusions and irregular tooth form often pose not only functional but also aesthetic problems for the patients. The presented case describes the correction of both problem areas.

Case report This case report presents a 61-year-old female patient with mandibular asymmetry, moderate Angle class III, anterior crossbite and edge-to-edge bite, anterior open bite and pronounced crowding, as well as their correction by means of a completely customized lingual appliance and extraction of a lower incisor. Subsequent conservative reshaping of some anterior teeth was done by the dentist. In order to optimize the aesthetics in the area of the extraction gap, the orthodontist temporarily applied a pontic in this area.

Conclusions In suitable cases, a mandibular incisor extraction can support the treatment of an asymmetry as well as a moderate Angle class III with a frontal edge-to-edge and/or cross bite. Even complex cases can be treated successfully and predictably with a completely customized lingual appliance.
