Bildgebung zur forensischen Altersdiagnostik bei Jugendlichen und jungen Erwachsenen

Bei immer mehr ausländischen Jugendlichen und jungen Erwachsenen ohne gültige Ausweispapiere wird bei vermutlich falschen Altersangaben eine forensische Altersdiagnostik angefordert. In diesem Beitrag werden Möglichkeiten und Grenzen der bildgebenden Verfahren bei lebenden Jugendlichen und jungen Erwachsenen dargestellt.

Kernaussagen
  • Die komplexen Anforderungen an die forensische Altersdiagnostik können nur durch ein interdisziplinäres Vorgehen erfüllt werden.

  • Bei Vorliegen einer Rechtsgrundlage für Röntgenuntersuchungen ohne medizinische Indikation (z. B. § 81a StPO, § 42f SGB VIII) sollten eine körperliche Untersuchung, eine Röntgenuntersuchung der Hand, eine zahnärztliche Untersuchung mit Anfertigung eines Orthopantomogramms sowie ggf. eine CT-Untersuchung der medialen Klavikulaepiphysen kombiniert werden.

  • Weder ein ausgereiftes Handskelett noch eine abgeschlossene Mineralisation der dritten Molaren erlauben einen zweifelsfreien Nachweis der Vollendung des 18. Lebensjahrs.

  • Während die Ossifikationsgeschwindigkeit maßgeblich vom sozioökonomischen Status beeinflusst wird, ist bei der Beurteilung der Mineralisation der dritten Molaren die ethnische Zugehörigkeit der zu begutachtenden Person zu beachten.

  • Da die objektive Befunddokumentation sonografischer Untersuchungen für Nachbegutachtungen problematisch ist, sollten in der Altersdiagnostik 2 in der Skelettsonografie erfahrene Untersucher das jeweilige Stadium übereinstimmend beurteilen.

  • Prinzipiell kann die Knochenreifung auch mit MRT beurteilt werden. Sobald genügend Studien zu deren Validierung vorhanden sind, kann die MRT für den Einsatz in der Praxis der Altersbegutachtung empfohlen werden.
