Influence of lipid emulsion, enriched by the fish oil, at the level of blood cytokines in the patients with severe stroke: first results

Rationale: Study changes of level of blood cytokines in the patients with severe stroke with and without prescribe to of lipid emulsion, enriched by the fish oil.

Methods: 20 patients with severe stroke (8 men and 12 women), aged from 33 up to 86 years old, located on treatment in ICU, were randomized to two groups. In the patients from the first group on the background of total parenteral nutrition were prescribed of the lipid emulsion “Lipoplus“ (B. Braun), enriched by the fish oil, on 250ml daily within five day. All these patients were obtained daily 12–14g of linoleic acid (ω-6), 1,2–2,8g of alpha-linolenic acid (ω-3) and 2,1–4,3g of eicosa-pentaenoic and docosahexaenoic acids (ω-3), 50mg of alpha-tocopherol and 75mg of ascorbyl palmitate.

In the patients of the second group the parenteral nutrition with and without application of standard lipid emulsions (“Lipofundin MCT/LCT“, B. Braun) also were prescribed. At the beginning of research and on sixth day in all patient identified the content in serum blood following cytokines: IL-2, IL-4, IL-6, IL-8, IL-10, IFN-γ, GM-CSF, TNFα (Bio-Plex, “Bio-Rad“, USA).

Results: The cytokines content in the blood serum of the patients first group, receiving the parenteral nutrition with inclusion of lipid emulsions, enriched by the fish oil, it has been significantly lower, than in the patients of the second (control) group. Specifically this concerned cytokines of IL-4, IL-10, TNFα, IFN-γ.

Conclusions: Lipid emulsion, enriched by the fish oil, substantially hold down the cytokine answer and the pro-inflammatory organism reaction of the patients with severe stroke in ICU.
