Diagnostik des hypophysären Mikroadenoms

Zusammenfassung

Die Diagnostik des hypophysären Mikroadenoms stellt für den Radiologen eine besondere Herausforderung dar: Ein kleinvolumiges Organ, das in anatomisch und teilweise auch funktionell sehr enger Nachbarschaft zu einer Vielzahl von Nachbarstrukturen liegt, muss mit hoher Orts- und Kontrastauflösung abgebildet werden, um die oft nur wenige Millimeter großen Adenome zu erfassen.
Als radiologische Methode der Wahl hat sich eindeutig die MRT als Bildgebungsstandard etabliert. Spezielle Techniken wie die dynamische MRT oder die seitengetrennte venöse Blutentnahme aus dem Sinus petrosus inferior nach Hormonstimulation (Stufenkatheteruntersuchung) sind zusätzliche Hilfen bei der Suche nach einem Mikroadenom. Anatomische Varianten ohne Krankheitswert können dabei die Differenzialdiagnose erschweren.

Abstract

Radiologic diagnosis of microadenomas of the pituitary gland is challenging since the pituitary gland is a very small-volume organ in close neighbourhood to many eloquent structures. Furthermore imaging necessitates high contrast and topographic resolution not to miss the often very subtle microadenomas.
Magnetic resonance imaging is the modality of choice providing multiplanar high contrast images of the pituitary gland and its adjacent structures. Special techniques like dynamic contrast enhanced imaging of the pituitary gland and bilateral inferior petrous sinus sampling are additional techniques to localize microadenomas. Additionally, anatomic variations can aggravate differential diagnosis.

• Die Hypophyse ist meist breiter als lang und im maximalen Durchmesser < 9 mm groß. Nach der Geburt, in der Pubertät und in der Schwangerschaft kann sie größer sein. Sie wiegt bei Männern zwischen 300 und 800 mg, bei Frauen zwischen 450 und 1000 mg. Etwa 75 % des Volumens werden von der Adenohypophyse eingenommen.

• Bei den Hypophysenadenomen wird zwischen Mikro- (< 10 mm) und Makroadenomen (> 10 mm) und zwischen hormonaktiven und -inaktiven Adenomen unterschieden. Inzidentalome sind zufällige, bei einer MRT-Untersuchung des Schädels gefundene Hypophysenadenome (meist Mikroadenome) bzw. Hypophysen, die auf über 10 mm Durchmesser vergrößert sind.

• Für die Bildgebung der Hypophyse ist die MRT die Methode der Wahl. Die Standarduntersuchung besteht aus einer nativ koronaren und sagittalen T1w SE-Sequenz (Schichtdicke < 3 mm). Zystische Läsionen sind sehr gut in koronaren, T2w SE-Sequenzen nachweisbar. Koronare und sagittale Sequenzen nach Kontrastmittelgabe (halbe Dosis) können die diagnostische Sensitivität weiter erhöhen. Jede Untersuchung der Hypophyse wird durch ein axiales T2w Bild durch das gesamte Gehirn ergänzt. Die CT ist für ergänzende Informationen über die knöchernen Strukturen oder bei kontraindizierter MRT sinnvoll.

• Makroadenome führen in > 94 % zu einer Vergrößerung der Sella, d. h. das Fehlen einer sellären Auftreibung spricht gegen ein Makroadenom. Mikroadenome erscheinen in der Standarduntersuchung mehrheitlich nativ hypointens und bleiben dies auch nach Kontrastmittelgabe. Indirekte Zeichen eines Mikroadenoms sind die einseitige Absenkung des Hypophysenbodens, die einseitige Anhebung des Diaphragma sellae und die Verlagerung des Hypophysenstiels zur kontralateralen Seite. Entscheidend für die Diagnostik des Mikroadenoms ist eine zusätzliche dynamische Untersuchung mit schnellen, repetitiven koronaren T1w Sequenzen.

• Die Stufenkatheteruntersuchung wird bei einem zentralen Morbus Cushing eingesetzt. Dabei wird seitengetrennt Blut aus dem Sinus petrosus inferior entnommen, um den asymmetrischen Anstieg des ACTH-Wertes nach CRH-Stimulation nachzuweisen.

• Wichtige Differenzialdiagnosen des Mikroadenoms sind die Rathke-Zyste (intrasellär, zwischen dem Hypophysenvorder- und -hinterlappen), das Kraniopharyngeom (häufig mit Zysten und Verkalkungen), die intraselläre Arachnoidalzyste und Tumoren bzw. Metastasen.
