Introduction

Scientific interest in natural-orifice transluminal endoscopic surgery (NOTES) continues to increase. A record number of abstracts on NOTES were presented at this year’s Digestive Diseases Week. A search for “NOTES” revealed 104 hits among this year’s submitted abstracts. In total, 78 abstracts related to NOTES procedures, of which 5 covered video case presentations. The remaining 73 reports have been classified under “Access and closure” (n = 25), “Indications for NOTES” (n = 19), “Training, education and health economics” (n = 6), “Infection and immunity” (n = 5), and “NOTES procedures in humans” (n = 6). Twelve abstracts have been classified under “New devices and tools.” The number of abstracts devoted to NOTES was thus double the number in DDW 2007. However, NOTES procedures are still determined by basic research in vitro or in animal models. Clinical applications in humans are very few. Nevertheless, many new aspects have been highlighted, and useful devices have been developed and studied that have the potential to facilitate not only NOTES but also endosurgery in general. The question will be which techniques, indications, and devices will disappear and which will become integrated into clinical practice.
