Zusammenfassung

Das Konzept einer gelenkübergreifenden Fixation mit achsengeführter Bewegung in der Gelenkachse hat sich für das Ellbogengelenk als vorteilhaft für die frühfunktionelle Nachbehandlung komplexer osteoligamentärer Verletzungen nach operativer Versorgung erwiesen und erlaubt auch nach komplexen knöchernen und ligamentären Rekonstruktionen eine rasche postoperative Nachbehandlung zur Vermeidung der Nachteile langfristiger Transfixation. Experimentelle Ansätze für derartige achsenkinematische Führungen mit externer Transfixation bestätigen die Möglichkeit einer Anwendung auch für das Handgelenk, Fingergelenke und das obere Sprunggelenk, die eine wesentlich kompliziertere Achsenkinematik besitzen, ohne dass hier bisher aber ausreichende klinische Resultate erzielt werden konnten.

Abstract

Postoperative transfixation of the elbow joint with a hinged device after ORIF shows advantages in the full-term treatment of complex osteoligamentous injuries and allows for early functional treatment after complex reconstruction of the bone and ligaments without the complication of arthrofibrosis. Biomechanical investigations on the more difficult kinematics also confirm the possibility for using hinged devices at the wrist, finger and ankle joints. Only few clinical results for the latter types of treatment are available to date.
