Zusammenfassung

Die Manifestation pankreatitisassoziierter Nekrosen in der Leistenregion als akute druckdolente Schwellung stellt ein äußerst seltenes Ereignis im Verlauf einer akuten Pankreatitis dar. Im vor­liegenden Fall erfolgte bei einem 61-jährigen Patienten eine explorative Laparotomie unter dem Verdacht auf ein Pankreaskopfkarzinom. Wegen lokaler Inoperabilität wurde der Eingriff nach histologischer Sicherung beendet. Postoperativ entwickelte der Patient eine akute inguinale Schwellung. Unter dem klinischen und sonografischen Verdacht einer inkarzerierten Inguinalhernie ­erfolgte die erneute operative Intervention. Hier zeigte sich eine akute nekrotisierende Pankrea­titis mit ausgedehnten tryptischen Nekrosen und Abszedierungen mit Ausbreitung bis ins kleine Becken und schließlich in den Leistenkanal. Die folgende Falldarstellung, der nach unserer Kenntnis erste berichtete Fall über die Manifestation ­einer postoperativen nekrotisierenden Pankrea­titis als akute Schwellung der Leistenregion, schildert den atypischen Verlauf und verdeutlicht die differenzialdiagnostischen Schwierigkeiten.

Abstract

Necrotising pancreatitis presenting as an acute inguinal swelling is an extremely rare condition. We report on a 61-year-old male who underwent explorative laparotomy for pancreatic tumour. Intraoperatively, the diagnosis of a locally advanced carcinoma of the head of the pancreas was confirmed histologically. During the postoperative course, the patient developed an acute inguinal swelling, which was suspected to be an incarcerated hernia. Intraoperatively, exploration of both abdomen and inguinal canal revealed an acute necrotising pancreatitis. This case is, to the best of our knowledge, the first case report on a postoperative necrotising pancreatitis masquerading as an incarcerated inguinal hernia in an adult male, and the diagnostic problems are discussed.
