Rezidivierende Makrohämaturie - Spätkomplikation nach Hüfttotalendoprothese

Zusammenfassung

Urologische Komplikationen nach totalem endoprothetischen Hüftgelenksersatz sind selten. Leitsymptom einer Hüftprothesenlockerung 8 Jahre nach Implantation war in dem hier beschriebenen Fall eine rezidivierende Makrohämaturie, bedingt durch eine Druckschädigung und eine Fremdkörperreaktion der Blasenwand auf Abriebprodukte der implantierten Polyäthylenpfanne.

Differentialdiagnostisch sollte bei Patienten mit Hüfttotalendoprothese und Makrohämaturie auf eine Komplikation der oben beschriebenen Art geachtet werden, zumal die Hüfte zumindest teilweise ohne weiteren Aufwand im Ausscheidungsurogramm beurteilt warden kann.

Abstract

Urologic complications following total hip replacement are rare. In the case reported herein eight years after implantation of a total hip prosthesis main symptom of loosening was gross haematuria. Reasons were first a mechanic irritation of the bladder wall by protrusion of the acetabulum and methylmetacrylate into the pelvis and second an inflammatory tissue response caused by small Polyethylen particles shorn off the prosthesis with wear.

As a conclusion in patients with total hip replacement occurence of haematuria can be assigned to complications described above. Diagnosis is possible without greater difficulty considering that the hip region can - at least partly - easily evaluated on routine intravenous pyelogram.
