Zusammenfassung

Die interstitielle Zystitis ist ein Krankheitsbild unklarer Genese, das im Spätstadium oftmals nur durch radikale Blasenersatz- bzw. Erweiterungschirurgie zu behandeln ist. Neben einer Fibrosierung der Blasenwand liegt eine Störung der Antiadhärenz-Aktivität der Blasenoberfläche vor. Durch Modifikation der Antiadhärenz-Aktivität bei oraler Pentosan-Polysulfat-Medikation ließ sich im Spätstadium der Erkrankung nur bei einem von 11 Patienten eine sichere klinische Verbesserung erreichen. Die eingeschränkten Miktionsvolumina konnten nicht gesteigert werden. Therapiebedingte serologische hämostaseologische und zytologische Veränderungen wurden in keinem Fall beobachtet. Durch Kombination der oralen Pentosan-Polysulfat-Medikation mit einer Dilatationstherapie bei Schrumpfblasenbildung waren in allen Fällen rasche klinische Verbesserungen und eine bleibende Steigerung des Miktionsvolumens zu erzielen. Urodynamisch war bei einer Steigerung des Schwellenwerts qualitativ keine Veränderung der Blasencompliance zu beobachten. Das risikoarme Kombinationsverfahren bestätigte seine Effektivität und empfiehlt sich daher als alternative Behandlungsmethode vor dem Einsatz radikaler chirurgischer Maßnahmen in der Behandlung des fibrotischen Endstadiums bei interstitieller Zystitis.

Abstract

Chronical interstitial cystitis is a disease state for which no cause has been defined. Concerning the treatment of the final state the only chance consists in radical surgical procedure. Fibrosis of the muscular layer and deficiency of the anti-adherence activity of the surface are the main histopathological features. Trying to improve the anti-adherence activity by oral medication of pentosan-polysulfate, our treatment of the final state of disease was successful in one of eleven patients only. Side effects, such as serological, haemostasiological or cytological changes were never seen. Quite the contrary. We achieved reliable long term results by distending the bladder in combination with adjuvant pentosan-polysulfate medication. These are defined by the increase of voiding volume, the decrease of nycturia and the disappearance of pains. Concerning urodynamic investigations we found an increase of capacity while the quality of bladder compliance did not change. We recommend the almost riskless combination-therapy as an alternative of invasive surgery concerning the final state of interstitial cystitis.
