Die urethrale Rekonstruktion mit freiem Blasenmukosatransplantat

Zusammenfassung

Bei 20 Kindern im Alter von 2 Œ-13 Jahren wurde ein freies Blasenmukosatransplantat zur Rekonstruktion der Urethra verwendet. Die Indikation umfaßte 18 schwere Hypospadien (10 primšre Formen, 8 mehrfach voroperierte Patienten) und 2 Epispadien. Die Schleimhaut heilte in alien Fällen vollständig ein, zeigte aber an der Glansspitze eine Tendenz zur Wucherung, die bei 7 Patienten eine Revision des Meatus urethrae erforderte. Diese Komplikation ist durch zusštzliches Einnähen eines zirkulären Vorhautstreifens am distalsten Ende des Transplantates vermeidbar.

Abstract

A free graft of bladder mucosa was used to construct a neourethra in 20 children. There were 18 patients with severe hypospadias (10 primary cases with insufficient preputial skin and 8 who had previous failed surgery for hypospadias) and 2 patients with epispadias. In every case the graft itself healed satisfactory. However revision of the meatus was required in 1 patients due to pouting of the mucosa at the tip of the glans. A combined tube with additional circumferential preputial skin at the tip is therefore recommended.
