Renale Hämatome nach ESWL - Verlaufsbeobachtungen

Zusammenfassung

Bei 1257 Patienten mit 1511 Behandlungen wurde von Dezember 1983 bis Oktober 1985 bei 19 Patienten (1,5%) ein intrarenales bzw. subkapsuläres Hämatom festgestellt. 17/19 Patienten hatten eine, 2 Patienten zwei ESWL-Behandlungen. 10 Patienten wurden konservativ behandelt, 2 Patienten mußten nephrektomiert werden, bei 6 Patienten erfolgte eine perkutane Punktion des sub-kapsulären Hämatoms, 1 Patient kam infolge einer foudroyant verlaufenden gramnegativen Sepsis ad exitum. 12 von 16 auswertbaren Patienten konnten im Hinblick auf Morphologic und Funktion der behandelten Niere zwischen 3 und 21 Monaten nachuntersucht werden. Im CT war bei allen Patienten eine vollständige Rückbildung des Hämatoms nachzuweisen. Bei 2 Patienten fanden sich jedoch morphologische Veränderungen der Niere.

Bei 1 Patientin wurde 14 Monate nach der ESWL eine signifikante Verschlechterung der Nierenfunktion in der 131 J-Hippuran-Clearance, offenbar durch narbige Um-wandlung des Hämatoms, beobachtet.

Abstract

Between December 1983 and October 1985, ESWL was performed 1511 times on 1257 patients, 19 of whom (1.5%) developed intrarenal or subcapsular hematoma. 17 of the 19 patients had undergone ESWL treatment once, 2 of them twice. Subsequently, 10 patients were managed conservatively, 2 patients required nephrectomy, 6 patients were treated by percutaneous puncture of the subcapsular hematoma, and 1 patient died of a fulminant gram-negative sepsis.

Morphology and function of the treated kidney were checked in 12 of the 16 patients available to follow-up at between 3 and 21 months. In all patients, CT established complete dispersion of the hematoma. However, 2 patients exhibited morphological changes in the kidney. In one case, a significant deterioration of renal function was revealed by the 1311-Hippuran clearance 14 months after ESWL which had obviously been caused by transformation of the hematoma into scar tissue.
