Zusammenfassung
Wir berichten über einen 27jährigen Patienten, der sich 2,5 Jahre nach Melanomexstirpation über der linken Scapula mit adjuvanter Chemotherapie und 1,5 Jahre nach Lymphknotendissektion der linken Axilla wegen Lymphknotenmetastasierung mit dem klinischen Bild einer zunehmenden Raumforderung im Bereich der rechten Niere bei uns vorstellte. Sonographisch und computertomographisch wurde nach einem Flankentrauma ein perirenales Hämatom diagnostiziert. Aufgrund einer akuten retround intraperitonealen Massenblutung wurde eine Woche später eine Notfallaparotomie notwendig. Hierbei fanden sich u.a. eine Melanommetastasierung in den rechten Nierenhilus sowie in das perirenale Fett mit Arrosion der Vena cava als Blutungsursache.
Abstract
We report on a 27-year-old patient, who was admitted because of an increasing space occupying process in the region of the right kidney, 2,5 years after exstirpation of a melanoma over the left scapula with adjuvant chemotherapy and 1,5 years after lymph node dissection of the left axilla because of lymph node metastases. After a trauma of the right flank sonography and computed tomography showed a pararenal haematoma. Because of acute retro- and intraperitonal bleeding a laparatomy was necessary one week later. Intraoperatively we found a metastasis of the melanoma in the hilus of the right kidney and the pararenal fat with arrosion of the vena cava as the origin of the bleeding.
