Zusammenfassung
Es wird über 13 Patienten mit nichtseminomatösen Hodentumoren im Stadium II C und III berichtet, die sich alle durch extrem große Tumormasse und exzessive Tumormarkererhöhung auszeichneten. Nach Chemotherapie beobachteten wir bei 7 Patienten keine Normalisierung der Tumormarker und bezeichneten diese Gruppe als Chemotherapieversager. 6 Patienten einer zweiten Gruppe sind nach den Kriterien der Tumormarker Chemotherapie-Responder. Insbesondere bei den Patienten der ersten Gruppe führten wir nach erweiterter vergeblicher Chemotherapie ausgedehnte Operationen mit kurativer Zielsetzung durch. Dennoch verstarben alle Patienten dieser Gruppe innerhalb von 6 Monaten am Tumor. Alle Patienten der zweiten Gruppe überlebten. Die Metastasenchirurgie hat sich in den Fällen als sinnlos erwiesen, bei denen chemotherapeutisch keine Normalisierung der Tumormarker zu erreichen war.
Abstract
We report about 30 patients with nonseminomatous testicular cancer stage II C and III who we saw from 1979 to 1985 at the Urologic Clinic of the University of Hamburg. All had extremely elevated tumor markers and large tumor mass.
After chemotherapy 7 of the patients had no normalization of the tumor markers and we call this group chemotherapy-resistent. A second group of 6 patients however, are chemotherapy-responders according to the normalization of their tumor markers. Especially in the first group of patients we did extensive surgical treatment to cure these patients. However, all these patients died with tumor progression within 6 months. The patients of the second group all survived. We concluded that extensive surgical therapy is not indicated in those patients with nonseminomatous testicular tumor who had no normalization of tumor markers after repeated chemotherapy.
