Große extraperitoneale Blasenskrotalhernie mit vesikorenalem Reflux beidseits

Zusammenfassung

Blasenskrotalhernien sind selten und werden in über 80% aller Fälle erst intraoperativ diagnostiziert, wenn die Blase irrtümlich als Bruchsack reseziert wird. Wegen der dann möglichen Komplikationen - wie z. B. Urinfistel und Sepsis - ist eine präoperative Diagnose erstrebenswert. Die Sonographic des Skrotums und der Harnblase ermöglicht eine rasche Diagnose, die letztlich mit einem Zystogramm bestätigt wird. An einem Fallbeispiel werden die Diagnostik und Therapie der verschiedenen Formen der Blasenskrotalhernie diskutiert. Jede große Skrotalhernie sollte zum Ausschluß einer Blasenhernie mit Ultraschall untersucht werden.

Abstract

Scrotal hernias containing urinary bladder are rare and 80% of the cases are diagnosed during operation when the bladder mistaken for sac has been resected. In these cases the complications can be as farreaching as urinary fistula and sepsis. For this reason a preoperative diagnosis is desirable. The ultrasound of scrotum and bladder permits a rapid diagnosis which may be confirmed by cystogram. Using one case report as an example the diagnostic method and therapy of the different forms of bladder scrotal hernia are discussed. Every large scrotal hernia should be subjected to ultrasound examination to exclude the possibility of bladder herniage.
