Sonographische Volumetrie präoperativ behandelter Wilms-Tumoren-Ergebnisse bei 59 Kindern

Zusammenfassung
In einer Studie zur Therapie der Wilms-Tumoren in der Bundesrepublik Deutschland (1980-1988) war die präoperative Behandlung mit Actinomycin D + Vincristin (15-22 Tage) von Kindern über zwei Jahren der Stadien III-V und von besonders großen Tumoren des Stadiums II Teil des Behandlungsprotokolls. 108/373 Kinder wurden präoperativ behandelt, von 59 lag eine kontinuierliche sonographisch ermittelte Volumenbestimmung der Tumoren vor. Bei 22 erfolgte ausschließlich eine zytostatische, bei 37 zusätzlich eine radiotherapeutische Vorbehandlung. Die Tumorvolumina waren am Ende der präoperativen Therapie in der Gesamtgruppe 55 % des Tumor-Ausgangsvolumens, der Behandlungseffekt galt in entsprechender Größe auch für histologisch hochmaligne Tumoren und war besonders ausgeprägt bei bilateralen Wilms-Tumoren. Die präoperative Behandlung erleichtert den operativen Eingriff und führt oft zu einer gewissen Reduktion der postoperativen therapeutischen Intensität, wesentlich auch zu einer Reduktion der Spätmorbidität. Daher ist die Vorbehandlung wie in der vorliegenden Studie individualisiert, aber nicht generell durchzuführen. Sie dürfte für 30 % aller Patienten sinnvoll sein. Auf eine präoperative Radiotherapie kann zukünftig verzichtet werden.
Abstract
In a comprehensive study for the treatment of Wilms tumors within the Federal Republic of Germany (1980-1988), preoperative treatment with actinomycin D plus vincristine for 15-22 days in children older than 2 years with stage III-V disease or with very large stage II-tumors was part of the treatment protocol. 108/373 children in the study had a preoperative treatment. In 59, a continuous follow-up of pretreat-ment and preoperative sonographically determined tumor volumes could be obtained., 22 of the children had a preoperative chemotherapy only, whereas 37 had an additional simultaneous radiotherapy (12-15 Gy in 2 weeks). The tumor volumes at the end of the preoperative treatment were 55 % of the initial volumes on the average of the whole group. The treatment effect was similar in histologic standard types of Wilms tumors and in high grade malignancy tumors. Bilateral Wilms tumors responded extremely favorable to preoperative treatment. Preoperative treatment had a positive effect upon extent and difficult of the radical tumor-neph-rectomy as well as upon the subsequent postoperative therapeutic intensity and upon late, treatment - related morbidity. Preoperative treatment of Wilms tumors should not be done in all children, but be individually chosen as proposed in the German Wilms tumor study of 1980. Preoperative radiotherapy, when added to the chemotherapy, does not further improve the cytoreductive effect of the drugs and should be abandoned.
