Zusammenfassung
Vor einer Operation sind Patienten in hohem Maße Stress und innerer Anspannung ausgesetzt.
Eine präoperative Medikation (Prämedikation) ist darauf abgestimmt, diesen Stress
durch Anxiolyse und Sedierung reduzieren. Hierfür werden aufgrund ihrer guten anxiolytischen
Wirkung, einer exzellenten Verträglichkeit und nur weniger Kontraindikationen am häufigsten
Benzodiazepine eingesetzt. Zur Unterstützung des Ein– und Durchschlafens beginnt die
Prämedikation in der Regel am Vorabend mit der Einnahme eines mittellang– oder langwirksamen
Benzodiazepins. Das Standardmedikament für den Operationstag ist das kurzwirksame
Benzodiazepin Midazolam, unter dessen Wirkung der Patient angstfrei und schläfrig
in den Operationstrakt eingeschleust werden kann. Die Wirkung der zur präoperativen
Medikation eingesetzten Medikamente beginnt verzögert. Daher ist es von großer Bedeutung
die Prämedikation zum richtigen Zeitpunkt zu verabreichen, damit der Patient auch
tatsächlich im Wirkoptimum der applizierten Substanz gegen den präoperativen Stress
abgeschirmt wird. Obgleich Midazolam nicht für alle Situationen die ideale anxiolytische
Prämedikationssubstanz am Operationstag ist, konnte mit Ausnahme von einigen speziellen
Indikationen in Vergleichsuntersuchungen für andere Substanzen bisher kein entscheidender
Vorteil aufgezeigt werden
Abstract
Prior to an operation, patients experience a high degree of stress and internal tension.
Preoperative drug administration (premedication) is intended to reduce these stresses
through anxiolytic and sedative effects. On account of their good anxiolytic actions,
there excellent tolerability and only few side effects benzodiazepines are most frequently
used for this purpose. To support the induction and continuation of sleep, premedication
is usually initiated in the evening before operation with the administration of a
benzodiazepine with middle– or long–term activity. The standard drug for the day of
operation is the short–acting benzodiazepine midazolam, under the action of which
the patient may be transferred to the operation room in a drowsy and calm state. The
onset of action of drugs applied as preoperative medication is delayed. Thus, it is
of major importance that the premedication be administered at the appropriate time
point so that the patient really is shielded fro preoperative stress at the time of
the drug's optimal effect. Although midazolam is not the ideal substance for all situations
where an anxiolytic premedication is needed, with the exception of a few special indications,
no decisive advantages over other drugs have been demonstrated in comparative trials.

Im Rahmen der routinemäßigen präoperativen Medikation (Prämedikation) sind insbesondere
eine Reduktion von Angst und Stress erwünscht.

die Standardsubstanzen für die Prämedikation.

Die orale Medikation gilt heute beim erwachsenen Patienten als Goldstandard der Prämedikation.

Auch bei ambulanten Patienten muss grundsätzlich das Bedürfnis nach Anxiolyse und
milder Sedierung unterstellt werden.

Die verfügbaren Benzodiazepine wirken qualitativ ähnlich.

Die Wahl des Benzodiazepins ergibt sich aus der Halbwertszeit sowie dem Abbauweg unter
Umgehung des Phase–I–Stoffwechsels (keine wesentliche Einschränkung bei Leberinsuffizienz).

In der Regel werden zur vorabendlichen Prämedikation mittellang– und langwirksame
Benzodiazepine eingesetzt (Ein– und Durchschlafförderung).

Am OP–Tag ist Midazolam aufgrund des raschen Wirkeintritts und der kurzen Wirkdauer
das Standardmedikament zur Prämedikation.

Anwendungsbeschränkungen für Benzodiazepine ergeben sich unter anderem bei bestehendem
Schlaf–Apnoe–Syndrom und bei Einschränkung der neuromuskulären Funktion (z.B. Myasthenia
gravis).

Neuroleptika müssen aufgrund der weitgehend fehlenden anxiolytischen Wirkung als Reservemedikamente
angesehen werden.

Kinder erhalten Midazolam vowiegend rektal.

Um den Patienten effektiv gegen präoperativen Stress abzuschirmen, muss Midazolam
zum richtigen Zeitpunkt gegeben werden (enges Zeitfenster für das Wirkoptimum).

Um Überdosierungen zu vermeiden, ist bei alten Patienten häufig eine Dosisreduktion
oder Titration („Start low – go slow”) der Substanzen angezeigt.

Obgleich das Themengebiet „Prämedikation” kaum mehr als problematisches Thema angesehen
wird, ist eine Substanz mit einem Wirkprofil, das allen Anforderungen gerecht wird,
derzeit nicht verfügbar.
