Die Transuretero-Pyelo-Uretero-Kutaneostomie

Zusammenfassung

Es wird eine neue Variante einer supravesikalen Harnableitung vorgestellt, bei der beide Nieren ohne Darmvermittlung mit Hilfe einer Tandem-Anastomose zwischen einem Harnleiter und dem kontralateralen Nierenbecken über ein (intubiertes) Harnleiterstoma Abfluß finden. Diese Methode vermeidet Nachteile der Transuretero-Uretero-Cutaneostomie (TUUC) und der Transuretero-Pyelo-Nephrostomie (TUPN). Sie bietet bei geringen Eingriffs- und Ableitungsrisiken mehr an Lebensqualität als eine bilaterale Nephrostomie oder Uretero-Cutaneostomie. Sie eignet sich daher als Alternative zur palliativen Nephrostomie bei Patienten mit günstiger Prognose und/oder geringeren Risiken sowie als Alternative zu anderen definitiven Ableitungen bei Patienten mit ungünstiger Prognose und/oder größeren Risiken.

Abstract

A new variant of supravesical urinary diversion is presented in which both kidneys are drained with one abdominal ureteral stoma (if necessary intubated) by means of a tandem anastomosis between one ureter and the contralateral pyelon without mediation of the bowel. This method avoids the disadvantages of the transuretero-uretero-cutaneostomy (TUUC) and transuretero-pyelo-nephrostomy (TUPN). It offers more than a bilateral nephrostomy or uretero-cutaneostomy in terms of the quality of life and the low risk involved. It is a favourable alternative to palliative nephrostomy in patients with a good prognosis and/or low risk as well as a good alternative to other definitive diversions in patients with a poor prognosis and/or higher risks.
