Zur Problematik der Doppelgipfel-Wellenform bei den frühen kortikalen somatosensorisch evozierten Potentialen

Summary

The presence of a double peak waveform in the primary cortical complex of the somatosensory evoked potentials is a rare finding. In this study we investigated 9 patients (mean age ± SD: 31.3 ± 11.3 years) with this phenomenon of the two subcomponents of the N20 response after stimulation of the median nerve. The results of the heterogenous patient group showed that the generation of the second subcomponent is for patients with severe head injury not pathognomonic, that it could be reversibel, and that medication induced effects are presumable responsible for that phenomenon.

Zusammenfassung

Das Auftreten einer Doppelgipfel-Wellenform beim kortikalen Primärkomplex der somatosensorisch evozierten Potentiale gilt als seltener Befund. Untersucht wurden im Rahmen dieser Studie neun Patienten (mittleres Alter 31,1 ± 11,3 Jahre) bei denen das Phänomen der zwei Subkomponenten der N20-Antwort nach Stimulation des N. medianus auftrat. Die Ergebnisse zeigten anhand eines heterogenen Patientengutes, daß die Entstehung der zweiten Subkomponente für Patienten mit Schädel-Hirn-Trauma nicht pathognomonisch ist und reversibel sein kann, und daß vermutlich medikationsinduzierte Effekte dafür verantwortlich sind.
