Summary
Proximal demyelinisation is a common pathophysiological feature in acute inflammatory demyeli-nating polyneuritis, chronic inflammatory demyelinating polyneuropathy and in multifocal motoric neuropathy. The diagnosis of this peripheral nerve disorder with classical electroneurography is not always successful. We demonstrate the data of motor evoked potentials and somatosensoric evoked potentials as a diagnostic tool in these immunneuropathies. Evoked potentials are more reliable than classical electroneurography. Motor evoked potentials are more sensitive than SEP. A proof of proximal demyelinisation is earlier possible with evoked potentials. We recommend the use of this diagnostic tool in polyneuropathy.
Zusammenfassung
Gemeinsames pathophysiologisches Merkmal des akuten Guillain-Barré-Syndroms, der chronischen Polyneuritis und der multifokalen motorischen Neuropathie mit Leitungsblock ist die Demyelinisierung, die häufig im proximalen Abschnitt des peripheren Nerven lokalisiert ist. Neurophysiologisch sind Demyelini-sierungen dieser Immunneuropathien mit der klassischen Neurographie nicht immer zu erfassen. Wir zeigen auf, daß der Einsatz der motorisch und somatosensorisch evozierten Potentiale in dieser Krankheitsgruppe zuverlässiger, oft früher, mitunter auch eindeutiger wegweisende Befunde erbringt. Dabei sind die motorisch evozierten Potentiale empfindlicher als die SEP. Dies entspricht auch der klinischen Manifestation dieser Immunneuropathien, bei denen motorische Ausfälle dominieren.
