Gefahren der suprapubischen Blasenpunktion nach Gefäßprothesenoperation

Zusammenfassung:

Die Therapie der Wahl beim akutem Harnverhalt aufgrund einer Harnröhrenstriktur ist die Anlage einer suprapubischen Zystostomie. Dieser Eingriff ist jedoch bei einer subkutan über die Blasenregion verlaufenden Gefäßprothese und einer gleichzeitigen antikoagulativen Therapie problematisch. Anhand eines Patienten mit klinischem Verdacht auf einen axillo-bifemoralen Bypass wird gezeigt, daß durch den gezielten Einsatz der dopplersonographischen Untersuchung der Blasenregion das Risiko einer Gefäßprothesenpunktion minimiert werden kann.

Abstract

The therapy of choice in patients with an acute urinary retention due to urethral stricture is a suprapubic cystostomy. This routine urological procedure can be extremely hazardous in cases with a subcutaneous vessel protheses crossing the bladder region and a concomitant anticoagulation therapy. Herein, we discuss the management of this rare clinical situation in patient with an axillo-bifemoral vessel prosthesis using the color duplex sonography for exact localization of the prosthesis.
