Zusammenfassung:

Anhand eines Fallberichtes wird über die kapilläre Chemoembolisation beim Nierenzellkarzinom mit einem in die Vena Cava eingebrochenen Tumorthrombus (Initial-stadium T3 Nx M0 V2) berichtet. Dabei wurde zur Embolisation Ethibloc® in Kombination mit Mitomycin C (7,5mg/10mg MMC) und 40%iger Glukose verwendet. 14 Monate nach der Chemoembolisation kam es zum Rezidiv, welches mit Polyoinylalkohol erfolgreich nachembolisiert werden konnte. Fast vier Jahre nach der ersten Embolisation zeigte sich in CT-Kontrollen eine deutliche Größenreduktion des avitalen Resttumors und des Vena cava-Thrombus als Zeichen der therapeutischen Effektivität dieser minimal invasiven Therapieform.

Abstract

Capillary chemoembolisation of tumors in the treatment of renal cell carcinoma with vena cava infiltration (T3 Nx M0 V2) is discussed on the basis of a high risk patient. This patient had been embolized using Ethibloc®/Mitomycin C (7.5mg/10mg MMC) in combination with 40% glucose as occlusion medium. Fourteen months after embolisation a recurrence of upper pole arteries was observed and successfully reembolized using polyvinyl-alcohol. Almost 4 years after the first embolisation, follow-up CT scans demonstrated massive shrinkage of the avital residual tumor and vena cava thrombus, indicating the effective, minimally invasive management of this disease.
