CT findings in subarachnoidal haemorrhage (SAH)

Summary

In a retrospective study the CT scans of 138 patients with the clinical diagnosis of SAH were reviewed. CT was highly sensitive in detecting blood in the CSF spaces during the 3 days following SAH, with decreasing accuracy correlated to the time interval between SAH und CT examination. Clinical state on admission and CT findings were closely related, as were the localisation of detectable blood and the site of source of bleeding. Whereas blood clots in the basal cisterns, above the convexities, and intracerebrally, as well as the finding of a brain oedema, were significantly correlated to the time of survival, hydrocephalus and ventricular haemorrhage had no bearing on the survival time.

Zusammenfassung

In einer retrospektiven Studie wurden die Computertomogramme von 138 Patienten mit der klinischen Diagnose einer Subarachnoidalblutung befundet. Während der ersten drei Tage nach der Subarachnoidalblutung war das Computertomogramm eine hochsensitive Methode zum Nachweis von Blut in den Subarachnoidalräumen; die Treffsicherheit nimmt mit zunehmendem Abstand vom Zeitpunkt der Blutung ab. Klinischer Befund bei Aufnahme und CT-Befunde waren eng miteinander korreliert; es bestand auch eine gute Übereinstimmung zwischen der Lokalisation des nachweisbaren Blutes im CT und der Lokalisation der Blutungsquelle. Während der Nachweis von Blut in den basalen Zisternen, über den Konvexitäten und intrazerebral mit der Überlebenszeit der Patienten signifikant korreliert waren, spielten der Nachweis eines Hydrozephalus und einer Ventrikelblutung im Hinblick auf die Prognose keine Rolle.
