The extreme lateral approach to thoracic disc herniations: technique and preliminary results

Summary

The extreme lateral approach to the thoracic disc spaces produces minimum disruption of the normal spinal anatomy, avoids retraction of the spinal cord, and preserves the intercostal neurovascular bundle. It is achieved by removal of about 5 cm of the rib which has its insertion at the disc level, if necessary associated with partial removal of the transverse process, followed by partial pediculotomy and a limited lateral rachotomy (vertebral body ressection), which permits the opening of the spinal canal exclusively ventral to the intervertebral foramen. The initial results of 6 patients, operated in up to four discs at the same occasion, are promising.

Zusammenfassung

Ein extrem lateraler Zugang zu den thorakalen Zwischenwirbelscheiben minimiert notwendige Eingriffe in die normale spinale Anatomie, verhindert eine Retraktion des Rückenmarks und hält die interkostalen neurovaskulären Stränge intakt. Man erreicht dies, indem man die fünf distalen Zentimeter vom dorsalen Rippenende entfernt, dessen Verbindung mit der Wirbelsäule in Höhe der Bandscheibe liegt. Falls notwendig, wird dies mit einer partiellen Wegnahme des Processus transversalis verbunden, auf die eine partielle Pedikulotomie und eine begrenzte laterale Rhachiotomie (d.h. Resektion des Corpus vertebralis) folgt. Der Spinalkanal wird dabei ausschließlich ventral vom Foramen intervertebralis geöffnet. Die ersten Behandlungsresultate von 6 Patienten, bei denen in einer Sitzung bis zu vier Bandscheiben operiert wurden, sind vielversprechend.
