Intracavernous giant fusiform aneurysm of the carotid artery treated with Gianturco coils*

Summary
A 30-year-old man with a giant fusiform aneurysm of the intracavernous segment of the right carotid artery, clinically revealed by right painful ophthalmoplegie with exophthalmos, was treated by closure of the carotid artery in the neck with two paediatric Gianturco coils. The aneurysm, which had already shown partial spontaneous intraluminal thrombosis on CT and angiography, was no longer filled and its volume progressively decreased, with almost complete recovery of the oculomotor deficits. Serial CT and angiography performed seven months after treatment demonstrated the reduction in size of the aneurysm, its exclusion from the circulation and a good supply of the intracranial vascular bed, without the necessity of an extracranial-intracranial by-pass.
Zusammenfassung
Ein 30j ähriger männlicher Patient mit einem fusiformen Riesenaneurysma des intrakavernösen Segments der rechten Karotisarterie, klinisch manifest durch eine rechtsseitige schmerzhafte Ophthalmoplegie mit Exophthalms, wurde mittels Verschluß der A. carotis externa durch zwei pädiatrische Gianturco-Stahlspiralen behandelt. Das Aneurysma, das bei der computertomographischen Untersuchung bereits eine spontane intraluminale Thrombose aufgewiesen hatte, war daraufhin nicht mehr gefüllt; sein Volumen nahm ständig ab und die Ausfälle des Okulomotorius waren schließlich fast vollständig behoben. Die sieben Monate nach der Behandlung vorgenommene serielle Computertomographie und Angiographie zeigten, daß es von der allgemeinen Zirkulation ausgeschlossen war und daß die Blutversorgung des intrakraniellen Gefäßbettes auch ohne extrakraniellen-intrakraniellen Bypass sichergestellt werden konnte.
