Extradural Spinal Meningiomas

Summary
Extradural spinal meningiomas are infrequent. The relevant clinical literature and the present case histories demonstrate that purely extradural meningiomas do not essentially differ from the common intradural types. They show the same histology, the same frequent location in the thoracic spine and the well-known sex preponderance of female patients. During Operation, an extradural meningioma may easily be mistaken for a spinal metastasis. Intraoperative histology is, therefore, decisive for adequate surgical treatment. The resection of the tumour should include a generous excision of the adjacent dura.
Zusammenfassung
Rein extradurale spinale Meningiome sind selten. Das einschlägige Schrifttum wie die eigenen Fälle zeigen, daß sich die extraduralen spinalen Meningiome ansonsten nicht von den gewöhnlichen intraduralen Meningiomen unterscheiden. Sie gleichen sich in der histologischen Natur, in der den Thorakalbereich bevorzugenden spinalen Lokalisation und in der weiblichen Geschlechtsdisposition. Da intraoperativ leicht das extradurale Meningiom mit einem malignen Tumor (Metastase) verwechselt werden kann, ist für die adäquate chirurgische Behandlung die perioperative Schnellschnittdiagnose entscheidend. Die Tumorresektion sollte eine großzügige komplette Duraresektion miteinschließen.
