Abstract
We report on the oedema of the dorsal hand resp. on the global swelling from the hand
to the distal forearm. It is mostly one-sided, being observed with patients suffering
of rheumatoid arthritis. This phenomenon is not rare but till now scarcely noticed.
It is remarkable that five of nine cases observed in the past few years belonged to
chronic psoriatic polyarthritis. Probably it is a case of lymphatic oedema that could
develop after a synovial rupture in the area around the elbow-joint. More often the
cause is unknown. The peculiar accumulation in psoriasis suggests an analogy to psoriatic
dactylitis. We repeat and extend the description of the latter.
Zusammenfassung
Wir berichten über das nicht so seltene, bislang aber noch kaum beachtete Phänomen
des Handrückenödems bzw. der meist einseitigen Globalschwellung der Hand bis zum distalen
Unterarm bei chronischer Polyarthritis, wobei von 9 in den letzten Jahren beobachteten
Krankheitsfällen auffälligerweise fünf einer chronischen Polyarthritis psoriatica
zugehörten. Wahrscheinlich handelt es sich um ein entzündliches Lymphödem, das nach
Synovialruptur im Bereich des Ellenbogengelenks entstehen kann, häufiger aber ohne
ersichtlichen Ursachennachweis verbleibt. Die eigentümliche Häufung bei Psoriasis
legte eine Analogie zur Daktylitis psoriatica nahe, deren Beschreibung wir hier wiederholt
und erweitert haben.
