Synovektomie der MP - und PIP-Gelenke

Abstract

Indication, technique and results of MP and PIP joint synovectomy are presented according to the literature. The significance of early synovectony is emphasized. A new technique for synovectomy of the MP joints is created. By temporary oblique dissection of the ulnar collateral ligament it is possible to approach the hitherto inaccessible palmar parts of the joint. An elongation of the ulnar collateral ligament is possible, avoiding the risk of increased volar subluxation of the base of the proximal phalanx.

Zusammenfassung

Anhand der Weltliteratur und eigener Erfahrungen werden Indikation, Technik und Ergebnisse der Synovektomie der MP- und der PIP-Gelenke dargestellt. Die Notwendigkeit der Frühsynovektomie wird betont. Für die MP-Gelenke wird eine neue Operationstechnik mit schräger Durchtrennung des ulnaren Seitenbandes angegeben, mit der die üblicherweise nicht erreichbaren palmaren Gelenkpartien gut Synovektomiert und die ulnaren Seitenbänder verlängert werden können-ohne Verstärkung der volaren Subluxation der Grundgliedbasen.
