Zusammenfassung

Während einer ausgedehnten Adenovirus-Epidemie wurden 150 Patienten mit durch Typ 8 bedingter Keratokonjunktivitis randomisiert prospektiv behandelt. Hierbei wurden folgende Behandlungsmuster angewendet: Monotherapie mit viruzidem Jod; Kombinationstherapie von Interferon Augentropfen mit viruzid wirksamem Jod oder virustatisch wirksamem Trifluorthymidin; symptomatische Therapie mit Vasokonstriktor; lokale Steroidapplikation mit begleitender Antibiotikaabdeckung. Der Erkrankungsverlauf unter Therapie wurde regelmäßig alle 3-4 Tage dokumentiert und unter Berücksichtigung der jeweiligen Symptomausprägung bewertet. Der Krankheitsverlauf wie auch eine vergleichende Gegenüberstellung von Krankheitsdauer, Häufigkeit von Keratitiden oder Nummulibildung sprechen für den Einsatz der Monotherapie mit dem viruziden Jod in PVP (Betaisodona®) als Trägersubstanz. Die klinische Anwendung des Interferons bei Keratokonjunktivitis epidemica hat sich in der Kombinationstherapie nicht bewährt. Steroide sollten nur in besonderen Komplikationsfällen, z.B. bei Auftreten einer Iritis oder von Pseudomembranen, kurzfristig angewendet werden. Als Monotherapie können sie weder eine Keratitis verhindern noch die Zahl vorhandener Nummuli reduzieren. Die durchschnittliche Erkrankungsdauer unter Applikation lokaler Steroide war im Vergleich zur Monotherapie mit viruzidem Jod deutlich verlängert.

Summary

150 patients with adenovirus type 8 infection where treated at random, in a prospective study. Based upon a quantifiable conjunctivitis severity score we tried to find out where there are differences in treatment. The best results were seen using polyvinylpyrrolidon-iodine (Betaisodona®) although it could not prevent totally subepithelial corneal infiltrates. The combination of exogenous interferon alpha with polyvinylpyrrolidoniodo-drops or trifluorothymidin-drops was less successful. We could not show any prophylactic effect of interferon on uninflamed fellow eyes. Treatment with vasoconstrictor did not show any therapeutic or prophylactic potency. This group of patients must be seen as a control group and the results of effectice therapy should significantly differ from the results in this group. Topical corticosteroids should be reserved for severe symptomatic cases and those with iritis and pseudomembranous conjunctivitis. Giving topical corticosteroides in combination with antibiotics we did not find any influence on the incidence of subepithelial keratitis or the number of corneal infiltrates. The mean duration of acute keratoconjunctivitis using this therapy was longer than the mean duration in the control group with vasoconstrictor.
