Abstract

Eight children with either panhypopituarism, severe growth hormone deficiency or neurosecretory dysfunction for growth hormone with a common history of umbilical cord encirclement around the neck underwent magnetic resonance imaging of the pituitary gland and the hypothalamus. Panhypopituitarism was associated with small to absent adenohypophysis, narrow or absent stalk and ectopic neurohypophysis whereas patients with partial GHD showed normal anatomical structures. Etiology of the endocrinopathy could be a primary malformation of the pituitary gland, an apoplexy of the gland or disturbation within the hypothalamus. We believe that the latter cause is predominant in umbilical cord encirclement.

Zusammenfassung

Bei 8 Kindern mit Panhypopituitarismus oder gestörter Wachstumshormonsekretion, welche bei Geburt eine Nabelschnurumschlingung mit Asphyxie aufwiesen, wurde eine kernspintomographische Untersuchung der Hypophyse und des Hypothalamus durchgeführt. Während bei Kindern mit Panhypopituitarismus die Adenohypophyse sehr klein war oder fehlte und die Neurohypophyse ektop lag, war sie bei Patienten mit partiellem Mangel an GH normal. Als Ursache der Endokrinopathie kommen Mißbildungen der Hypophyse, eine Apoplexie der Hypophyse oder funktionelle, nicht darstellbare Veränderungen im Hypothalamus in Frage. Bei der Nabelschnurumschlingung muß in erster Linie an hypoxämische Schäden in Hypothalamus und Hypophyse gedacht werden.
