Die Korrektur von posttraumatischen Fehlstellungspseudarthrosen der Tibia mit dem Taylor Spatial Frame

Zusammenfassung

Hintergrund: Die Korrektur von Fehlstellungspseudarthrosen im Bereich des Unterschenkels ist bei ausgeprägter Fehlstellung und kompromitierten Weichteilen nur graduell möglich. Der Taylor Spatial Frame (TSF) ist ein hexapod basierter Ringfixateur, der eine simultane Korrektur von mehrdimensionalen Fehlstellungen ermöglicht. Anhand von 10 Patienten mit einer Fehlstellungspseudarthrose der Tibia werden die Anwendungsmöglichkeiten und Ergebnisse der geschlossenen Pseudarthrosenbehandlung mit dem TSF dargestellt. Patienten und Methode: 10 Patienten mit einer hypertrophen/oligotrophen Pseudarthrose der Tibia wurden zwischen 2003 bis 2006 mit dem TSF korrigiert. Das Durchschnittsalter der Patienten betrug 43 Jahre (min. 12, max. 73). Bei 5 Patienten lag eine offene Fraktur vor, bei 5 war die Fraktur geschlossen. 5 Patienten hatten eine hypertrophe, 5 Patienten eine oligotrophe Pseudarthrose. Die Angulationsfehlstellung betrug durchschnittlich 21,9° (min. 5, max. 44). Die Korrektur der Fehlstellung erfolgte bei 9 Patienten geschlossen, bei einem Patienten war eine Metallentfernung vor Korrektur erforderlich. Die Nachuntersuchungszeit betrug 26,2 Monate (min. 10, max. 44). Ergebnisse: Alle Fehlstellungen konnten mit einer durchschnittlichen Korrekturzeit von 23,1 Tagen (min. 5, max. 43) geschlossen korrigiert werden. Die mittlere Transportgeschwindigkeit betrug 1,3 mm/Tag (min. 0,5, max. 2), die Fixateurtragezeit 158,5 Tage (min. 89, max. 300). Eine chirurgische Knochenstimulation oder Augmentation wurde bei keinem Patienten durchgeführt. Als Komplikationen fanden sich eine Restachsabweichung von 5° valgus bei einem Patienten, ein Pininfekt und eine Ermüdungsfraktur. Schlussfolgerung: Der TSF ermöglicht eine dreidimensionale Korrektur von Fehlstellungen ohne zeitaufwendige Fixateurumbauten. Er eignet sich im Besonderen zur geschlossenen und graduellen Korrektur von hypertrophen/oligotrophen Fehlstellungspseudarthrosen.

Abstract

Background: Correction of severe tibial deformity due to a non-union of the tibia is best achieved by a slow gradual correction which allows the compromised tissue to adapt. The Taylor spatial frame (TSF) is a hexapod-based ring fixator that allows a simultaneous gradual correction of a multiplanar deformity through a virtual hinge. The aim of this study was to demonstrate the application of the TSF and the clinical outcomes of ten patients with a non-union of the tibia. Patients and Method: Ten patients with either a hypertrophic or atrophic non-union of the tibia were treated between 2003 and 2006 with the TSF. The mean age of these patients was 43 years (range: 12–73). There were 5 hypertrophic and 5 atrophic non-unions. Five of the ten fractures were initially open. The mean preoperative magnitude of the angulation deformity was 21.9° (range: 5–44). In nine patients, the correction was performed in a closed manner; one patient had hardware removed prior to the frame application. Results: In all ten patients, the deformities were corrected. The average duration of time that the frame was worn was 158.5 days (range: 89–300). The mean time for gradual correction was 23.1 days (range: 5–43) with an average correction speed of 1.3 mm/day (range: 0.5–2). No additional bone stimulation was performed. In terms of complications, one patient had a residual valgus deformity of 5°, one a pin tract infection, and one a stress fracture next to a pin hole. Conclusion: In patients with either a hypertrophic or oligotrophic non-union of the tibia and severe deformity, the TSF is a powerful tool to correct the deformity in a closed manner which preserves the remaining viability of the non-union.
