Abstract
47 cases of Pneumothorax (P) were encountered among 3679 consecutive patients of the neonatology unit of the Landes Kinderkrankenhaus Linz (= 1.3%). Of these 47 cases 38 occurred in artifically ventilated patients this amounts to 11.7% of all 325 ventilated newbornbabies during the study period. Only 9 Pneumothoraces (= 0.24%) occurred spontaneously.
Pneumothoraces in non ventilated patients were confined to the first few hours of life while the risk of pneumothorax is equally spread over the ventilation period in babies on respirators. Pneumothoraces which occurred in ventilated babies showed always tension signs while only 2 of the 9 spontaneously occurring pneumothoraces showed increased tensions. Transillumination of thorax was extremely helpfull as bedside test for diagnosis.
Zusammenfassung
Unter 3679 konsekutiven Patienten der Neonatologieabteilung des Landeskinderkrankenhauses Linz sind 47 Fälle (= 1,3%) von Pneumothorax aufgetreten. 38 Pneumothoraxe sind bei den 325 Beatmungsfällen (= 11,7%) der Studienperiode zu verzeichnen gewesen und 9 Pneumothoraxe entfielen auf die restlichen Patienten (= 0,24%) Spontanpneumothoraxe bei nicht beatmeten Patienten treten praktisch nur innerhalb der ersten Lebensstunden auf, während das Pneumothoraxrisiko bei beatmeten Kindern sich auf die ganze Beatmungszeit erstreckt.
Unter Beatmung sind nur Spannungspneumothoraxe aufgetreten, während dies ohne Beatmung nur bei zwei von neun der Fall war.
In der Sofortdiagnose hat sich die Thoraxtransillumination besonders beim Frühgeborenen ausgezeichnet bewährt.
