Abstract

In 61 patients (pts.) with biopsy proven osteogenic sarcoma a disease free survival rate of > 80% was obtained using T-7 chemotherapy. This can be explained by successful eradication of pulmonary micrometastases with aggressive chemotherapy. The prognosis for pts. < 12 years was significantly improved since a higher dose of Methotrexate was used for this younger age group. The histologic effect of preoperative chemotherapy on the primary tumor is of prognostic value for the outcome of the disease. Pts. with good histologic effect on the primary tumor (grade III and IV effect) show a 100% disease free survival rate, whereas pts. with poor histologic effect (grade I and II effect) have a 50% chance to develop metastases. The role of cis platinum in future therapeutic trials is discussed based on the experience with cis platinum in phase II trials.

Zusammenfassung

Mit dem T-7 Chemotherapie-Protokoll konnte bei 61 Patienten mit osteogenem Sarkom eine Remissionsrate von 80% nach 2 Jahren erreicht werden. Dies wird erklärt durch eine erfolgreiche Elimination von Mikrometastasen in der Lunge mit einer im Vergleich zu dem T-4/T-5 Protokoll intensivierten Chemotherapie. Bei Patienten < 12 Jahren konnte durch eine Erhöhung der Methotrexat Dosis auf 12g/m2 die Prognose signifikant verbessert werden. Der Effekt präoperativer Chemotherapie am Primärtumor hat entscheidende prognostische Bedeutung: Patienten mit hochgradiger Tumornekrose nach präoperativer Chemotherapie sind bislang alle erkrankungsfrei, Patienten mit minimalem Chemotherapieeffekt am Primärtumor haben eine 50% Chance, Metastasen zu entwickeln. Die Rolle von Cis-Platinum in der Chemotherapie des osteogenen Sarkoms wird diskutiert auf der Grundlage von Monotherapiestudien an Patienten mit metastasiertem, HDMTX resistenten osteogenem Sarkom.
