Abstract

In 384 healthy prematurely born infants, full-term newborns and older infants (conceptional age: 26 to 60 weeks; weight at investigation: 720 g to 5800 g) the flow in the anterior cerebral arteries was measured by pulsed dopplersonography. All infants were investigated with a 5 MHz probe (ATL Mark 600 and Acuson 128). The following parameters were measured from the flow profile: The maximal systolic velocity, the endsystolic and enddiastolic velocity as well as the mean flow velocity, the pulsatility-index and the resistance-index. For all parameters the relationship to the conceptional age and weight were analysed. All flow velocities increased with increasing conceptional age: There was an exponential increase of the flow velocities with the conceptional age, whereas the relationship between the weight and the flow velocities was linear. In contrast to this neither the pulsatility-index nor the resistance-index changed with increasing conceptional age and weight. As flow velocities in the anterior cerebral arteries increase with increasing conceptional age and weight both parameters must be taken into consideration when pathologic flow velocities are measured.

Zusammenfassung

Bei 384 gesunden Frühgeborenen, Neugeborenen und älteren Säuglingen (Konzeptionsalter: 26 bis 60 Wochen; Untersuchungsgewicht: 720 bis 5800 g) wurden mit Hilfe der gepulsten Dopplersonographie die Strömungsparameter in der A. cerebri ant. gemessen und Normalwerte erstellt. Alle Kinder wurden mit einem 5 MHz-Schallkopf untersucht und zwar bis 1986 mit dem ATL Mark 600 seit 1987 mit dem Acuson 128. Folgende Parameter wurden gemessen: Die maximale systolische, die endsystolische und die enddiastolische Flußgeschwindigkeit sowie die mittlere Flußgeschwindigkeit. Weiterhin wurde der Pulsatilitäts-Index und der Resistance-Index bestimmt. Für alle Parameter wurde die Abhängigkeit der Meßparameter vom Konzeptionsalter und Untersuchungsgewicht bestimmt. Mit zunehmendem Konzeptionsalter und Untersuchungsgewicht kam es zum Anstieg aller Flußgeschwindigkeiten: Für die Abhängigkeit vom Konzeptionsalter ergab sich ein exponentieller Anstieg, während für die Abhängigkeit vom Gewicht ein linearer Zusammenhang bestand. Demgegenüber zeigten weder der Pulsatilitäts-Index noch der Resistance-Index eine eindeutige Abhängigkeit vom Alter und Gewicht. Auf Grund der Zunahme der Flußgeschwindigkeiten mit zunehmendem Konzeptionsalter und Gewicht muß bei der Beurteilung pathologischer Flußgeschwindigkeiten immer das Alter und Untersuchungsgewicht mitberücksichtigt werden.
